Immunization Reminder for incoming 7th & 12th Graders
The Ohio Department of Health requires a Tdap Booster and a Meningococcal Vaccine before entry into 7th grade. All students must have a total of 2 Meningococcal vaccines before entry into 12th grade.
- INCOMING SEVENTH GRADERS AT LEBANON JUNIOR HIGH: Students who are currently in sixth grade are required to have a Tdap Booster and a Meningococcal Vaccine before going into 7th grade in August 2023.
- INCOMING SENIORS AT LEBANON HIGH SCHOOL: Students who are currently in 11th grade are required to have a total two Meningococcal Vaccines before going into 12th grade in August 2023.
Immunizations can be obtained from your family physician, the Warren County Health District, or area clinics and pharmacies. Immunization records must be turned into the school.
The Warren County Health District will be offering an Immunization Clinic for Lebanon students. The clinic will take place April 25 from 4 to 6:00 p.m. by appointment only at the Warren County Health District offices located at 416 S. East Street, Lebanon. Call 513-695-1468 to make an appointment.
