Deadline Approaching for Immunization Requirments

The Ohio Department of Health requires a Tdap Booster and a Meningococcal Vaccine before entry into 7th grade. All students must have a total of 2 Meningococcal vaccines before entry into 12th grade. Immunization records must be turned in or the student may be excluded from attending school.

SEVENTH GRADERS AT LEBANON JUNIOR HIGH: The due date to turn in proof of immunization or an immunization exemption* is September 6. By Ohio law, if proof of immunization or an immunization exemption is not received by the school by September 6, the student will not be able to attend school on September 7 and until the record is received.

SENIORS AT LEBANON HIGH SCHOOL: The due date to turn in proof of immunization or an immunization exemption* is September 7. By Ohio law, if proof of immunization or an immunization exemption is not received by the school by September 7, the student will not be able to attend school on September 8 and until the record is received.

Immunizations can be obtained from your family physician, the Warren County Health Department, or area clinics and pharmacies.

How to turn in proof of vaccinations or an exemption: drop off at our school buildings, mail it, or you may scan and email it to the school nurse: 

** Per Ohio Revised Code and school board policy, a student may be exempted from immunization if a parent or guardian objects for good cause, including religious conviction, or if there is a medical condition that prohibits immunization.
