Notes From The Nurse

The Ohio Department of Health requires immunization with the Tdap Booster and Meningococcal (MCV4) Vaccine before entry into the 7th grade.  If your student has not had these immunizations, please make arrangements for him or her to receive them as soon as possible.  Please send proof of these immunizations to the Junior High Nursing Office.  Proof of immunizations must be provided to the Nursing Office by the 14th day of attending school, or by Ohio law, your student will be excluded from school.  For additional information, please contact the school nurse at 513-934-5309 or the Warren County Health Department at 513-695-1468. If your child cannot receive these vaccinations due to medical reasons, religious convictions or reasons of conscience, please contact the school nurse. Per Ohio Revised Code and school board policy, a student may be exempted from immunization if a parent or guardian objects for good cause, including religious conviction, or if there is a medical condition that prohibits immunization. 

A Postural Screening Program will be conducted on all 7th grade students during the school year. The screening will help identify students with curvature of the spine called scoliosis or other possible spinal problems. A specialized nurse screens our students and if the student has been identified as having a possible spine curvature, a letter will be sent home to alert the parent to follow up with their primary care physician. If your student is currently under active treatment for a postural problem, or if you do not want your student screened, please contact the nurse at 934-5309 to complete and sign a form to be kept on file.

Only Ludens and Fruit Breezers cough drops are permitted at school without a medical form signed by a physician.

All medication kept in the nursing office must have an “Administration of Medication” form signed by a physician and parent. This can be found by clicking the link above or the main office.

If your child has a fever, please keep them home until they are fever free for 24 hours without the use of fever reducing medication.

 
 

Pre-Approved Absence Policy

If a student has to miss school due to a family obligations, the following procedures must be followed:

  1. The student must be traveling in the company of the parent or legal guardian.
  2. The student must notify the principal at least five (5) school days prior to the vacation and secure a vacation leave form. The form must be completed by the parent/guardian, signed by the principal and teachers, and returned to the attendance office before the vacation.
  3. Vacation requests will not be approved during state tests, the last week of school, or during the last week of the first semester, due to final exams.
  4. An 18-year-old student must follow all aspects of this policy.
  5. This policy may only be used once during the school year.
  6. It is the student’s responsibility to get homework assignments and make up any tests that were missed.  All work is due upon student return.
