BACK TO SCHOOL GUIDE - FORMS

For the 2024-2025 school year, we have partnered with FinalForms, an online forms and data management service, to help with the process of collecting forms and important information from families. 

FORMS ARE DUE PRIOR TO THE FIRST DAY OF SCHOOL.

Filling out forms annually ensures that we have current contact and emergency medical information, which requires a parent/legal guardian review and signature every year.  

STEPS TO COMPLETE STUDENT FORMS IN FINAL FORMS:

  1. Parents/guardians should have received an email from the district earlier this summer that contained account information. Follow the instructions in the email to confirm your account and complete the annual school forms for your student.

  2. If you need access to a computer, we welcome you to visit one of the stations set up at the District Office, 160 Miller Road, Monday through Friday, 8 a.m. to 4 p.m. 

  3. If you did not receive an email with account information, complete the Final Forms Help Request.
